Patient Measurement Report (Released January 2025)

The new Patient Measurement Report, a patient-specific report for a single calendar month is now available.  This PDF includes patient data organized by graphs, daily readings (table), and form submissions associated to that patient.

Data Included in the Patient Measurement Report

The Patient Measurement Report provides data for the patient for a single calendar month.  You may scroll to the end of this section to see screenshots of a sample report. The sections included in the report are:

• Header information
  The first page will include the clinic logo, name, and date and month that it was generated
• Footer information
  Each page will a footer with the patient name, date of birth, and timestamp at which the report was generated
• Basic patient information
  The first page includes the patient's name, gender, date of birth, and phone number
• Basic care management information
  A concise listing follows to note any current care management service enrollment, date of enrollment, and supervising provider name
• Current month blood pressure readings by category
  If blood pressure is included, a chart of blood pressure readings is shown that includes the count of readings, columns to show the number and percentage of readings that fall into each blood pressure category (hypotensive, normal, elevated, stage 1, stage 2, crisis), and count and percentage of readings for each category.
• Summary boxes for current and previous month
  For each measurement type included in the report, a brief summary box is then shown. Each summary box will show the minimum, mean, and maximum value for each reading for the current month and also the previous month, for a total of six data points.
• Year graphs
  Each measurement included will show a year graph of data, with each graph displaying each individual data point and a two week average.
• Current month daily measurements table
  A table is then shown that lists all individual measurements for the current month, displayed along with date and time recorded.
• Current month form submissions
  All form submissions made by or on behalf of this patient for the current month will be included.  Please note that the notes entries will not be included.

Patients for which the Patient Measurement Report can be generated

The Patient Measurement Report can be generated for all patients, regardless of whether or not they are or were enrolled in any care management services.

Report download options

The Patient Measurement Report can be downloaded in two ways:

• Patient Report
  This is a single, downloadable report for one patient for a specific month. It is downloaded directly from the patient’s chart.
• Bulk Report
  This option allows you to download reports for multiple patients or for multiple months for a single patient. It is ideal for bulk data review or reporting, and is accessed currently at Reports -> Care Management -> Patient Bulk Reports currently.

How to download an individual Patient Measurement Report from a patient chart

1. Access Patient Chart
   Access the patient's chart.  If you do not already have the Patient Chart open, you can use the 'Search Bar' above.
   
   
2. Click "Download Report"
   Within the Patient Chart, click on the 'Download Report' Button between the Call User and Patient Care drop-down.
   
   
   
   If you're in the new 'Monitoring Mode' view, the 'Download Report' button will be below the Patient Care/ Billing Code/ Timer (See the following article to learn more about Monitoring Mode - A New User Interface to View Patient Profiles).
   
   
   
   
3. Select "Patient Measurement Report"
   A modal will pop-up titled 'Download Report.' Within the modal you will have three 'Report Types' to select from.  Select the 'Patient Measurement Report' in the middle.
   
   
4. Select Desired Month
   Select the desired month to generate the report for by clicking under the 'Date' Selector.
5. Confirm Data Set
   You may restrict the data included in the report to only the most commonly-used data by checking the Core Data Only checkbox.  If you would to include all measurement data that is available for a patient, leave the box unchecked.  When the Core Data Only box is checked, it will include only the following measurements:
   1. Weight
   2. Blood Pressure
   3. Heart Rate
   4. Blood Oxygen
   5. Blood Glucose
6. Download Report
   Finally, click 'Download' at the bottom of the modal

 

How to download an multiple Patient Measurement Reports through Bulk Download

1. Access Care Management Reports page
   Go to the Reports link in the left hand menu
   
2. Access Patient Bulk Reports tab
   From this page, access the bulk report generation area by clicking the Patient Bulk Reports tab in the middle.
3. Start process to generate new bulk report
   Click the New Patient Report button in the upper left of the page
4. Select 'Patient Measurement Report' type
   From the options shown in the modal, click the radio button with the text Patient Measurement Report.
5. Select Report Options
   Before generating the report, you must define which reports you want to generate.  Please enter the following options:
   1. Search Clinic by Name or ID
      This is the clinic for which you want to generate patient reports from.  Lookup and select the clinic, searching by name or ID, and reports only for patients from this clinic will be included in the bulk download.  You select a clinic.
   2. Report Name
      The name you would like the bulk download file to be titled.  This has no impact on the data that is generated, but is used for your reference.
   3. Select which patients to include
      Once you select the clinic, two options for which patient(s) to include are presented:
      1. All Patients in Clinic: will include reports for all patients in the selected clinic
      2. Specific Patient: will include reports only for this selected patient.  This is most useful for when you want to download many months' reports for a single patient
         1. When this option is selected, an additional search box will be presented to search for and select the respective patient.
   4. Set dates
      Select the Start Date and End Date months for the reports to be generated.  To select only one month, set the same month for the start and end date fields.  If you different months are selected, reports will be downloaded for all months between the start and end months.
   5. Confirm data set
      You may restrict the data included in the report to only the most commonly-used data by checking the Core Data Only checkbox.  If you would to include all measurement data that is available for a patient, leave the box unchecked.  When the Core Data Only box is checked, it will include only the following measurements:
      1. Weight
      2. Blood Pressure
      3. Heart Rate
      4. Blood Oxygen
      5. Blood Glucose
6. Generate report
   Once filled, click 'Generate Patient Report' at the bottom of the modal
7. Wait for report download to become avaialble
   The Report will go into a queue and process the request. Once the Report is done processing, then the Status will change to 'Succeeded' with a 'Download File' link stating the quantity '1/1' there are of reports.  Please note that currently the files for download are limited to 100 reports each.  If more than 100 reports are generated for an export, multiple files will be generated and available for download.  For example, if 235 reports were generated, three files will be presented for download.  You must download all files to retrieve all generated reports

Viewing the Patient Measurement Report

Once the reports are downloaded, open the zip file or individual PDF report file.  In the case of the bulk file download, each PDF included in the zip file will be titled in the following format, each separated by an underscore:

• Month and Year of report (in YYYY-MM-DD format)
• First name of patient
• Last name of patient
• Organization/clinic ID
• Patient ID

All names and identifying information included in this guide are fictional.
