Weight Threshold Alert Deprecation

The Legacy Weight Threshold Alert is being deprecated, and the new Delta Change Alert type will be used in its place.  

Notable Changes

1. Alert Preference Configuration
   For any user that manages the alert preference configuration for a clinic, they will see the Legacy Weight Threshold alert will no longer be checked:
   but instead at least one Delta Change Alert - Weight alert preference set 
2. Alert -> Notification Page listing
   The alert -> notifications page will have two changes:
   1. The alert notification types will change from Weight Threshold alert type to Delta Change - Weight type.
   2. The Filter by Alert options for these alert notification types will change from Weight Threshold to Delta Change Alert.
      
3. Alert -> Patient Chart
   1. The patient chart -> alert card will have two changes:
      1. The alert notification types will change from Weight Threshold alert type to Delta Change - Weight type.
      2. The Filter by Alert options for these alert notification types will change from Weight Threshold to Delta Change Alert.
