Creating a Form

The Provider Dashboard has a lot of usages for Coaches & Clinics but, to realize it's full potential that data must be set up and gathered from somewhere which is where Creating a Form is handy so let's get to it!

Step #1:

Log into the Provider Dashboard for your clinic then, go to Digital Libary via the menu on the left.

Step #2:

In the Digital Library, Select the button labeled Create & Edit Forms.

Step #3

Now, go ahead and select the Create Form button on the right.

Step #4:

Give your Form a title, choose if you want to allow Addendums from your coaches, and set the Max Submission to one or unlimited for patient entries.
