It's possible to associate a Form with your Digital Libary Content for various purposes.
Step #1:
Start by going to your Digital Library from the side menu after logging into the dashboard.
Step #2:
With manage content chosen, select the button labeled Insert Form.
Step #3:
Select the form you wish to Insert, the name is set by default but, you can change it then give it a description and set if you want it public, private, or by package if available.
