Building a Form

After you create a form, you will need to Build your Form by adding fields to it so that you can begin to properly collect information from patients. 

Step #1:

First, log into the dashboard then select Edit Form for the form you wish to build.

Step #2:

By default, your form will have a basic Short Answer Field already on it to add your first question.

Step #3:

You can then add more questions using the + below the first one to Add Additional Questions.

Step #4:

If needed, you can Change the Question Type that is being asked.

Step #5:

You can also rearrange the Order of your form fields to organize it better.

Step #6:

Additionally, you can divide your form into sections if needed.

Step #7:

Another useful feature can force patients to have to fill in certain fields by marking it as Required.

Step #8:

If needed you can always Preview your form to see what it will look like.

Step #9:

Last but, not least and most importantly, don't forget to Save your form!
