Viewing your Submissions

Whenever a patient fills out your form you can find those submitted submissions by viewing the related Submission Listing.

Step #1:

Go to the Digital Library and select Create & Edit Forms.

Step #2:

Find the Form you wish to inquiry and select View Submissions where you can see a listing.

Step #3:

Optionally, you can also choose to View Submissions Individually.
