A one page Patient Clinic Report can now be downloaded from each patients' profile page. The report summarizes a patient's activity over a specific time period. The generated file is designed to be utilized as additional documentation for a patient when submitting claims for Remote Patient Monitoring.
Location and How to Download
A Download Report button is presented at the top of the 'Dashboard' tab of a patient's profile. This new button is available on all patient profiles.
When the Download Report button is clicked, a dialog is presented with options for downloading the report. Keep in mind that the report is composed of information for a single patient, and includes a summary of data collected over a specific time period.
- Start Date The starting date from which to include data calculated in the report
- End Date The ending date until which to include data calculated in the report
- Format The file format to download the report, either PDF or Excel
Description of Report
Both formats of the report (PDF or Excel) will contain the same information. The screenshots included below are from the PDF version, but small differences in presentation and formatting may exist between the two versions.
Dates of Service
The first item presented in the report is of the Dates of Service. These dates will correspond to the Start Date and End Date selected in the Download Patient Report dialog.
Patient Care Plan
This section applies to patients enrolled in Remote Patient Monitoring (RPM). If the patient has never had RPM enabled, some of these fields may be left blank. If RPM has been activated and deactivated multiple times for a patient, the data presented in this section relates to the most recent RPM activation and deactivation.
- Care Plan Type If RPM was enabled for a patient, it will show RPM as the care plan. Otherwise, it will be blank
- Status The current status of RPM for the patient, either Active or Inactive.
- Activation Date The date which RPM was activated for the patient.
- Target Deactivation Date The date RPM was projected or intended to be deactivated for this patient. This date would have been optionally selected by a provider. It corresponds to the anticipated date at which the patient was expected to achieve their goals as per the use of RPM. Depending on the activation date, this date may be in the past or future.
- Actual Deactivation Date The date RPM was most recently deactivated for this patient (if available).
- Reason for Deactivation The justification as to why RPM was turned off for a patient.
- Consent Obtained If the patient's consent to enable RPM for their account was obtained before RPM was activated. This question is asked when RPM is enabled for a patient's account.
- Face-to-face within 12 months If the provider and patient had a face-to-face meeting within the last 12 months. This question is asked when RPM is enabled for a patient's account.
- Device Supplied If a device used to electronically collect daily measurements was supplied to the patient. This question is asked when RPM is enabled for a patient's account.
- Device Education Supplied If instruction on how to use the supplied device was provided. This question is asked when RPM is enabled for a patient's account.
Patient Data
This information relates to patient activity. It provides starting and ending values for each metric or area, along with the amount and percent of change between those two values. In addition, it reports the count of unique days in which that data or data within that category was saved by the patient.
Each metric (Weight, BMI, Carbs, etc) is presented in a similar structure:
- Unit The unit of measurement used to record and track this metric
- Start The earliest recorded measurement for this metric, within the selected date range for this report
- End The last recorded measurement for this metric, within the selected date range for this report
- Unit Change The change in units between the start and end values in this row
- Percent Change The percent of change in units between the start and end values in this row
- Total Transmission Days The number of unique or distinct days, during the selected time period, in which data in this category was recorded.
Specific notes regarding each column are as follows:
- Weight
- Shown in either pounds of kilograms, as per the provider's profile preferences that generated the report
- If RPM is enabled for a patient, the Total Transmission Days also includes the unique/distinct days in which the automatic RPM notification to weigh in was received by the patient. To clarify, a specific day will contribute either 0 or 1 to the total count, and will be 1 if a specific day included with a recorded weight, the receipt of the automatic notification to weigh in, or a recorded weight and receipt of the automatic notification to weigh in.
- BMI
- If RPM is enabled for a patient, the Total Transmission Days also includes the unique/distinct days in which the automatic RPM notification to weigh in was received by the patient. To clarify, a specific day will contribute either 0 or 1 to the total count, and will be 1 if a specific day included with a recorded BMI, the receipt of the automatic notification to weigh in, or a recorded BMI and receipt of the automatic notification to weigh in.
- Supplements
- The unit of measurement is supplements per day. For a particular supplement, it will be recorded either 0 or 1 times per day. Further, it will be recorded as 1 only if the minimum daily target was recorded. For example, if a supplement is listed as intended to be taken 3 times a day, it will only be recorded as 1 if that supplement was taken at least 3 times on that day.
Provider Activity
This final segment reports on provider interactions with this patient. Data from any provider within the clinic is included in the numbers:
- Calls (Audio/Video)
- Patient Interactions The number of calls this patient has engaged in with any provider within the clinic over the selected date range.
- Time The total number of minutes the patient spent in audio/video calls over the selected time period.
- Data Monitoring
- Patient Interactions This number will always be 0
- Time The total amount of time any provider within the clinic spent viewing any portion of the patient's profile on the provider dashboard during the selected time period.
- Messages
- Patient Interactions The number of unique messages sent and received by this patient over the selected time period
- Time This number will always be 0
- Total
- Patient Interactions The sum of the first three rows' Patient Interactions values
- Time The sum of the first three rows' Time values, in minutes
