Beginning Wednesday, March 17, 2021, PDF versions of both blank forms and form submissions can be created on the dashboard.
Feature Details
PDF versions of a form can be generated for both blank forms and existing form submissions:
- PDF Heading
Each PDF will include the following information:
- Clinic Logo
The logo of the currently-selected clinic from the upper right hand side of the dashboard. - Clinic Name
The name of the currently-selected clinic from the upper right hand side of the dashboard. - Form Name
Will be displayed in bold centered text at the bottom of the header - Patient
For a form submission, this will show the name of the patient to which the form submission is associated. This means either the form was submitted by this patient in their mobile app, or the form was submitted by a coach on the dashboard for this patient. - Submitted By
The name of the user (coach or patient) that created this form submission. - Submitted On
The date and time of this form submission.

- Blank Form PDF
A PDF version of a blank form can be created by clicking the PDF icon in the Actions column of the listing of all forms. As a reminder, this listing if found in the Digital Library link in the lefthand menu, and by clicking the Create & Edit Forms tab. This will create a one or multiple-page PDF (depending on form length) with blank and unselected fields for all questions.
This blank PDF can be created regardless of whether form has any submissions. - Submission PDF
A PDF version of a form submission can be created by clicking that form (or the Submission Listing icon in the Actions column) from the listing of all forms. From there, locate a specific form submission to create a PDF for, and click the PDF icon in the Actions column. This will create a PDF that includes all of the responses for that specific submission.
